MEDICAL CERTIFICATION
I certify that I (or my child) am physically fit to participate in all activities at the Academy and have no medical condition that would prevent safe participation. I understand that it is my responsibility to consult with a physician before participating.
WAIVER AND RELEASE OF LIABILITY
In consideration for being permitted to participate in activities at the Academy, I, for myself, my heirs, assigns, personal representatives, and next of kin, hereby release and hold harmless:
Neiman Gracie Jiu Jitsu Academy, its owners, instructors, staff, agents, volunteers, affiliates, successors, and assigns from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, injury, or damage, including death, that may be sustained while participating in Academy activities, on or off-site, whether caused by negligence or otherwise.
RULES & CONDUCT
I agree to follow all safety rules, policies, and instructions provided by the Academy. I understand that failure to do so may result in suspension or termination of participation without refund.
